'Ekahi Care Management (ECM) aims to develop and deliver disease management, care management, and well-being programs. ECM is innovating healthcare here in Hawaii. New specialty clinic with the focus on diabetes management mixing technology with frequent interpersonal touches. Team oriented approach to move the total cost care needle in the right direction. 

ECM is seeking a Licensed Clinical Social Worker to provide behavioral health services to our Medicare population. The LCSW will work closely with the interdisciplinary team to provide a holistic approach to support participants along their wellness journey. The position is a part time position with the opportunity to evolve into a full time role as the participant panel grows.

Primary Responsibilities and Duties

  • Provide individual and group sessions. Session would be held either online, at the Ekahi facility or physicians office
  • Obtain and interpret all relevant screening, assessment and initial treatment planning information. Confirm the client's continued readiness for change
  • Assure the accurate and timely documentation of individual/group sessions
  • Facilitate psychoeducational support group sessions as needed for 'Ekahi patients
  • Coordination with insurance companies regarding reimbursement rates
  • Provides responsive customer service to referral sources. Maintain boundaries and flexibility in interactions
  • Facilitates a high level of participant satisfaction
  • Complies with HIPAA privacy and patient confidentiality policies and procedures
  • Provides educational in-service to Team members, as necessary


  • State License for Clinical Social Worker
  • Currently credentialed with Medicare and MDX - preferred
  • Experience working with the Medicare population
  • Minimum of 3-5 years experience
  • Experience in clinics in the start-up phase is preferred but not required
  • Personal interests in diabetes management
  • Excellent computer skills and entry-level experience with information system technology
  • Exemplary interpersonal and customer service skills
  • Excellent organization skills
  • Exhibits sensitivity to the needs of participants
  • Desire and ability to work in a patient-centered, team-oriented environment
  • A passion for health and promotion and wellness
  • Able to work independently, self-motivated and flexible to changing processes for efficiency
