Overview

'Ekahi Diabetes program and 'Ekahi Go aims to develop and deliver innovative disease management, care management, and well-being programs here in Hawaii.  A new specialty clinic with a focus on diabetes management and mixing technology with a team-oriented approach.

The Behavioral Health Specialist will provide individual and group therapy to patients to enable them to learn techniques to bring their disease under control, avoid potential complications, and bring about lasting improvements in quality of life.  The Behavioral Health Specialist will possess flexible thinking, excellent communication skills, the ability to examine, evaluate, and discuss the effectiveness of the care plan.  The Behavioral Health Specialist will be flexible and can have the ability to work collaboratively with clinic staff and patients and be dedicated to the quality of work, and performance monitoring.  This position is a contract position with the ability to evolve into a Full or Part Time role.

Essential Duties and Responsibilities 

  • Facilitate individual or group sessions.  The session would be held either virtually, at the 'Ekahi facility or physician's office
  • Obtain and interpret all relevant screening, assessment, and initial treatment planning information.  Confirm the client's continued readiness for change
  • Assure the accurate and timely documentation of individual/group sessions
  • Provide psychoeducational sessions as needed for 'Ekahi patients
  • Coordination with insurance companies regarding reimbursement rates
  • Provides responsive customer service to referral sources.  Maintain boundaries and flexibility in interactions
  • Facilitates a high level of participant satisfaction  
  • Complies with HIPAA privacy and patient confidentiality policies and procedures 
  • Provides educational in-service to Team members, as necessary

 Desired Qualifications

  • Psychologist, psychiatrist, licensed clinical social worker (LCSW), or other mental health professionals (licensed clinical nurse, nurse practitioner) with state licensure or state approval. Please refer to State Mental Health licensing requirements to ensure candidate eligibility
  • Current certification in BLS or CPR is required  
  • Computer literacy and electronic health record experience 
  • Experience and adeptness in working with groups and individuals  
  • Ability to facilitate a process of self-expression and active listening in a way that can create connection, community, and intimacy among group participants  
  • Ability to model and promote verbalization of feelings, personal use of self and an empathic listening presence  
  • Experience with individuals confronting chronic illness 
  • Experience working with the psychosocial issues that may arise when making lifestyle changes
  • Excellent public speaking skills and knowledge of adult education and coaching principles  
  • Excellent interpersonal communication skills with sensitivity to participant needs 
  • Desire and willingness to collaborate as a member of a high-functioning transdisciplinary team that supports personal transformation
  • Strong passion for holistic health and wellness
